Advance Directives for Healthcare

You can let your family and physician know what kind of treatment you want in the event you cannot speak for yourself.

You can also designate someone to make decisions on your behalf if you become incapacitated.

Other documents, including Comfort Care Only/Do Not Resuscitate and Medic Alert Bracelets or Necklaces give instructions to doctors, paramedics, nurses and other health care providers in emergencies. Forms must be completed by your physician. Forms may also be obtained from the Emergency Medical Services Section, State Department of Health.
